START WITH THE PROBLEM
A few useful details can get the conversation started.
The form asks for information a prospective client is likely to know:

Who should we talk with?
Where can we reach you?
Organization (optional)
What business or organization is this for?
What would you like to accomplish?
Tell us about the idea or problem.
What are you working with today? (optional)
If something already exists, tell us about it.
Anything else we should know? (optional)
Deadlines, constraints, concerns, or other useful context.
WHAT HAPPENS NEXT
Understand first. Decide what follows.
The first conversation is about understanding the need. That may uncover questions about users and existing processes. Constraints or opportunities may also become clearer.
Once the problem is better understood, the next step can be determined. That might mean defining a project or examining an existing system more closely.
It does not need to begin with a technology decision.
